Senior Grad Bash Form

GRAD BASH GOT APPROVED!
Our trip this year will be to Islands of Adventure and Universal Studios. In order to save your spot, you will need to fill out the field trip form, the custody form, and upload a picture of your parent’s driver’s license.
In order to fully save your spot, these documents MUST be turned in through the following Microsoft Form.
The form will close on 9AM- Wednesday, November 3rd. To be approved you need to have a 3.0 unweighted GPA, adhere to the 10/20 rule this school year, complete your fundraising requirement (this will be explained during the assembly on November 8), and complete the form.
PLEASE READ ALL OF THE REQUIREMENTS BEFORE SUBMITTING. IF YOU DO NOT MEET THE CRITERIA YOU WILL BE DENIED.
The cost of the trip is $315 per student. If eligible to attend the class trip, you will be added to OSP in order to pay. Payments will be accepted from December 1st until the 5th.
Before completing the form, you must print out and complete both the field trip form and the police custody acknowledgment form.
To the Microsoft Form, you must attach:
1. Your completed field-trip form
2. Your police custody form.
3. A copy of your parent’s driver’s license
